As of March 27, 2013, Diane Foley, APRN will be changing her status from a part time provider to a per diem provider for Wentworth Health Partners. Patients of Diane Foley, APRN are encouraged to continue their care with either Mark Berman, MD or Kathleen Reid, APRN who are both accepting new patients at Bellamy Health Center.
We are now a Wentworth Health Partners Practice! Wentworth Health Partners is a new formation of primary and specialty care practices that have come together as a partnership in order to better coordinate patient care throughout our system. You will begin to see our name listed as Wentworth Health Partners Bellamy Health Center!
After Hours Clinic
Wentworth Health Partners is pleased to announce our After Hours Clinic that offers appointments for acute/sick visits Monday through Thursday evenings and Saturday mornings. Appointments are available to established patients of any Wentworth Health Partners primary care practice. The After Hours Clinic is located at 10 Members Way, Suite 200 in the Wentworth Professional Building in Dover, NH. The hours are 4:00 P.M. to 8:00 P.M. Monday through Thursday and 8:00 A.M. to 12:00 P.M. on Saturday. Appointments can be booked three ways:
1. Book an Acute/Sick appointment through your regular service provider's office beginning at 12:00 P.M. for appointments that same evening. Saturday Acute/Sick appointments can be booked through your regular provider's office beginning each Friday at 12:00 P.M.
2. After regular business hours and Saturday mornings call your primary care provider's office and use the prompt for the nurse triage or provider on-call service to request an appointment time.
3. The After Hours Clinic Phone Line is available Monday through Thursday 4:00 P.M. to 8:00 P.M. and on Saturday from 8:00 A.M. to 12:00 P.M. at 603-609-6930.
Patient Portal
We now offer the NextMD Patient Portal! Sign up at the office today or download the Patient Portal Consent Form and return it to our office. The Portal is a secure website that allows access to the following:
- Request Appointments
- Request Prescription Refills
- Receive Test Results
- Ask general questions
ePrescribing Available
Wentworth Health Partners Bellamy Health Center is pleased to announce the availability of ePrescribing. E-Prescribing is a computer-based transmission and filling of a prescription between your provider and pharmacy. We encourage you to contact your pharmacy directly for refills to facilitate a faster turnaround time for refill requests.
Medicare Annual Wellness Exam
Wentworth Health Partners Bellamy Health Center is now providing Medicare Annual Wellness Exams. Please contact the practice if you have any additional questions regarding this exam and eligibility or to schedule your appointment. A Medicare Annual Exam Questionnaire should be filled out and brought with to your Annual Wellness appointment.
